For the safety of our children and youth within the Zero Collective, background checks are required for every volunteer and staff member serving in Children’s, Student, Worship Arts, or Connections Ministry. All checks are conducted through the Michigan State Police.

This form must be submitted and processed at least 48 hours prior to a volunteer’s first serving date in Children’s, Student, Worship Arts or Connections Ministry.

By completing this form, you authorize us to conduct a background check as needed for up to one year from the date listed on the form. All individuals 18 years of age or older must complete a background check before serving.
